Form LOA-001, Letter of Attestation for Lab Entry, shall be completed, signed and notarized by supplier when

equipment is intended for placement in AT&T test laboratories and the equipment does NOT meet Objective 1.11

of this document.

Per requirement 1.11 (Company name) hereby asserts, to the best of its knowledge, and pursuant conclusions

drawn from sound engineering judgment, that the equipment described below meets or exceeds electrical safety

and fire standards as detailed in UL 60950.
